Haven't seen it before. Not quite sure how it would happen. Added more purging of the remote-tracking branches prior to fetching though, which should hopefully prevent this.
refs/remotes is supposed to be empty unless Savage is in the process of doing a fetch/push, and there's only one PullRequestPusher actor, which should effectively serialize those operations.
