I've fixed the stream problem but the "bad from and until" test does not work.

I'm not sure if the tests are ok anyway winstonjs/winston#345
As soon as I fix the test I'll open a pull request

I'm setting up my couchdb transport using:

winston.add(require('winston-couchdb').Couchdb, { level: 'warn' });

I'm seeing multiple levels of logging in my couchdb (debug, info, etc.). It seems there's a related issue in winston on the levels not being correctly ordered, but I don't think this bug is affected by that one.

I've trawled through a few of the transport sources and thought perhaps we're not saving the level param so I added:

this.level = options.level;

modeled after winston-sns. This did not change behavior.
