Can't make multiple lists

When i make 2 Lists in a single modal it makes it a single radio box.

If i select for example "DKK" my option from selected "Month" will be removed and it will instead be "DKK".

some working sample would be nice

Hi,
I understand a lot has been done for dynamic form building, but would be nice if you could have a working sample, a walk through exercise.
