According to the webmention standard, webmentions for the same page should not be duplicated and instead overwrite old mentions if the source and target are the same. This is currently not the case. It appears that while this is the case for pending mentions. it is not the case for the full stored mentions.

To fix this, we need to change the following code to instead perform an update using upsert
