Hi,

1. index.php forwards to login.html (should be login.php as login.html does not exist)
2. login.php page comes up blank

Please fix so I can install and use,

A few issues which are fairly important which I have noticed;

• It uses mysql_ calls which should not be used in PHP anymore. They no longer exist in PHP 7+ e.g. here - use PDO and prepared statements
• You include some files multiple times from the same page like db.php gets included multiple times.
• Using md5 to hash passwords should no longer be done, have a look at password_hash
• Short tags are also not enabled by default on a lot of servers, you should use the full <?php
