I compiled your example and it seems that some hosts don't recognize plugin's ability to receive events, while others do. VST host, which was proposed in your tutorial doesn't recognize that the plugin can take any events at all. The midi icon in the plugin panel inside the host doesn't show any activity when I try sending some midi messages. I've tried attaching a debugger and it looks like the process_events function doesn't get called at all. On the other hand, I've opened the plugin inside Ableton Live and it worked fine, but it's very inconvinient to use Live to test and debug my plugin.

EDIT: Also the "Chain After" button on the plugin panel is greyed out in VST Host, looks like no inputs are being found for the plugin.
