Add Google Assistant "Service"

Since we use API.AI, we can forego the Alexa skill and have API.AI deployed automatically and turn it into a Google Assistant service.

Integrate with FHIR.

We have tried. Many have failed. This will allow us to create appointments and get doctor information automatically.

EXCEEDED_MAX_REPROMPTS

When EXCEEDED_MAX_REPROMPTS is triggered (the user does not respond multiple times), Alexa incorrectly assumes that the Skill response failed, where in reality we just didn't cover this case.
