Add h js files to front-end about madison

The JS files have been added, but the iframe being used needs to be included as a view. Right now it needs to be compiled from the python template into HTML so we can include it as a blade file.

The extension files can be built with bin/hypothesis extension development.ini https://hypothes.is/app, which will create a build folder with all of the necessary assets.

Ok I'll look into that

The host.js file needs to be changed to use a template as the 'app' ( line 32 ) instead of the iframe. I've been told that we can strip the 'tal' and 'metal' tags out of the hypothesis app template (/h/templates/app.pt) and use that, but actually running localhost:5000/app and grabbing what's there may work better.

We're no longer including the H code, but Annotator instead.
