Ugly. Reproduced by Dan Mills.
This seems like a regression if we must use "always" vs. "from visited".
I am pretty sure "from visited" has been working on iOS phone/iPad for myfavoritebeer.

To be clear, I reported this for the case where I was not accepting any cookies at all.

@vthunder's case was for when he was in the (default?) ipad setting of 'from visited'.
