In an ideal world, initexmf should disallow the wrong settings. And pdfTeX should not silently fail to produce correct output. The first problem is a MiKTeX issue which won't be fixed. The second problem is a pdfTeX issue which won't be fixed here (MiKTeX project). Please get in touch with the pdfTeX team, if you need a fix.
