just by looking at the code, there are some fundamental flaws with the Vulkan implementation:

• You're logging on vkQueueSubmit. This function just submits a command buffer, it does not correspond to one frame - what you're looking for is vkQueuePresentKHR.
• You're hooking Vulkan functions. This is generally a bad idea for a number of reasons, the biggest issue being that most Vulkan applications do not use the functions exported by vulkan-1, but only use vkGetInstanceProcAddr to retrieve function pointers directly from the driver.
