Rating starts with yellow stars instead of grey stars about starability

Hey @iFlash I've added an empty input to have no rating at all at the beginning in the new version. You can copy it from the README or index.html.

This fix solves the display problem, but unfortunately it means there's no way to simply make a star rating required with HTML5. As far as the browser is concerned, value="0" is still a valid value, so hitting Submit works fine. Even if semantically I treat it on the server side as though it was left out, that means a full client=>server=>client roundtrip to validate required fields.
