In some instances the URL of the tab may change, but the page is never actually reloaded, so a new message indicating the href change is never sent to context.js.

in these cases, the frameURL that replace.js is checking against will be outdated and the correct frame will never be found.

This is especially prevalent on youtube.com

content.js is run successfully (sending the appropriate href) when youtube.com is initially loaded. However, clicking a video changes the url to www.youtube.com/watch......, without actually rerunning content.js. So, replace.js is looking for www.youtube.com still.
