Missing setup information

The README lacks information about how to set up dynmenu.

My approach so far (using code from pull-request #2):

git clone <repo url> # to ~/.menu
cat menu.sql | sqlite3 main.sqlite
{ echo "BEGIN;"; dmenu_path|{while read c; do echo "INSERT INTO commands (cmd) VALUES ('$c');" ; done}; echo "END;" } | sqlite3 test.sqlite
sqlite3 main.sqlite
# change dmenu hotkey to ~/.menu/menu main

Commands that do not exist are never removed from the database

I am not sure if it is actually desirable to delete old and now non-existant commands, maybe not by default.

Keeping non-existant commands is a problem to me when a program is renamed but the old defunct command has been called a lot and thus will always be selected first.
