This is a great idea for a plugin and I look forward to using it.

Even though the Decisions plugin is now on the wordpress.org repo (congratulations), I don't expect to install it from there.

As you mention in the README, it makes a lot of sense to run this as an MU plugin. I recognize there are a number of ways to run a complete plugin from the mu-plugins directory, however it is markedly simpler to run a single file plugin from there.

Generally speaking, I don't like the idea of jamming all the functionality into a single php file but when the plugin footprint is so close to being a single file I think there would be value in doing so in this case.
