This is slightly different in format from the previous SAS source code used for comorbidity extraction, therefore put on hold.

Have included the code from Dr. Quan, with his permission, for sourcing some ICD-10 comorbidity maps.
Still using the original published ICD-9 maps for the other code, but see no need to update these unless anyone notices any problems. If so, please write the test case and reference the correct source!
