Split the hierarchy into a new module

At the moment the content module handles the hierarchy of the pages handled by the module. But by doing it internally in the module, it is impossible to combine elements from multiple modules into one coherent hierarchy.
In order to do so, the hierarchy will need to be managed externally.

Sorting by Related Pages results in no entries

When in the admin area, sorting on the Related Pages column (content_subs) results in an empty list. All other sortable columns are working.

Tested in ImpressCMS 1.4.2 - 1.4.4, and1.5.0 (new 2.0.0)

Ghost pages generated automatically

Over the times, the content overview gets populated with ghost pages.

make current user the author of a new page by default

When I create a page, the page creator is by default the first user alphabetically in the list of users on the site, not the user that is currently logged in. Make it the currently logged in user by default.
