Dublicate Map entries

To Reproduce
Steps to reproduce the behavior:

1. Wait for the bot to finish the query
2. Restart the bot
3. Let the bot query the server again
4. use !history
5. the same map will appear twice in the list

Expected behavior
Only display the map once

Show the "Last 10 Maps" section inverted (most recent map first) instead of most recent map last (current implementation)

• multiple game servers support (allow multiple History instances)
• multiple discord support (history per discord guild)
• link history to discord server
• allow multiple embeds per discord
