Use Synthea to create a cohort of 1000+ patients

• be sure to document build and run commands and put in the repo
• inspect synthetic dataset to ensure it has enough asthma-related phenotypes/codes
• include good asthma example dataset in the repo once we've found one
• output as both FHIR and CSV

After #1 and #2 have been worked out, we need to run:

• a database migration to set up mysql with the correct database tables and relationships for FHIR
• data population of the synthetic asthma dataset into mysql

Include definitions and commands to set up a FHIR instance, including a FHIR server and mysql database

PR should include:

• docker compose file to easily set up services
• Cloudformation templates for AWS-based deployments
