Problem importing metareviewers from csv about expertiza

This is related to issues #145 and #198, could be done in a single project including those issues.

This was addressed by E1776, which was merged, but I see a cryptic message,
"Sequence of columns in file:

1. Contributor
2. Reviewer
3. Metareviewers"

But the file contains only one column!

Importing meta-reviewers does work now, but it gives this evidently spurious error message after the import:
"The following errors were encountered during import.
Other records may have been added. A second submission will not duplicate these records.
undefined method `id' for nil:NilClass"
