Support Multiple Endpoint for GraphqlModule in Code-first Approach

One suggest by creating two different app, admin and shop would solve this endpoint problems but during bootstrapping, and in environments for config, there could be replication and conflicts for multiple apps.

Due to `Request Context` addition to service layer, there has been some problems for testing

Allow `Customer` to Create two or multiple `Address` in which one of them has to be default address.

Currently, Customer can only create one address due to unique constraint on customerId. If customer has two or multiple address, customer should be enforced to make single address default, which make defualtBillingAddress and defualtShippingAddress to be true.

Add `Permission` Field to Role Entity

Since SessionService check with getUserChannelsPermissions for Permssion, Role entity is in need of permission field. If u go with creating Permission entity, ManytoMany relation is favorable since one permssion such asCreateCustomer could belong to one or more role such as Administrator, Assistant Manager.
