Fix submission error on members page

Reproducing

  • Login to any club's account which already has members.
  • Try adding details like Govt. ID Card, Photo and hit submit.

Neither any form error is displayed, nor does the form get submitted.

Add CONTRIBUTING.md

Add a decent CONTRIBUTING.md file for enthusiasts to contribute to the project.

Will be based upon README.md.
