Handle request/response matching better

When sending requests from the client to the server we send an incrementing serial number to the server, and it sends back the same serial number on the response. We use this to match up our response to the callback to send it to.

If the serial number of the response is less than the serial number of the first pending callback, we should discard the response. If the serial number of the response is greater than the first pending callback we should discard the callback.

This is not fully implemented, because we also need to implement rolling-over the serial-number counter. Also we would need to send an error to the waiting callback so their thread is hung on an MVar forever.

Currently we never discard the first pending callback, under the assumption the server will never drop requests.

The function we need to modify is compareOpaque. RFC 1982 should help somewhat.

Handle expiring entries in cache

The silkapp branch of starling includes support for setting an expiration date in cache entries.

This should either be brought over as-is, or via new set/add/replace functions.

The memcached expiration parameter is weird, so we may also want to make it more friendly.
