Add an option to hide IP addresses

When using nginx proxy, the server's IP address will be displayed, which is not secure. It is recommended to add a switch option to select whether to display the logged in IP address.

Cleanup Icons

Migrate icons from code to CSS to improve plugin performance.

Exclude login, 2fa and password pages as they are isolated.

Documentation URL is hard-coded

You should use the definition in the file constants.php:

Bug: Documentation URL Does Not Work Since KB v1.2.25 Release

#10 was working fine until v1.2.25 was released and the URL string was changed in the core here: kanboard/kanboard@80c0734

As the string is pulled form the core, only show the link if the user has KB v1.2.25+ installed.

Ignore `localhost` as IP Address

Issue:

On the defaut login screen, at the very bottom of the login panel, I see “Your IP: 127.0.0.1”. I host kanboard behind a reverse proxy, so the web server only listens on 127.0.0.1/localhost
