These commits should make ripple-lib more efficient for long-running processes.

(The received_tx map seems to only be used for checking that a particular received transaction will only be handled once, esp. useful for multi-server). The default max of 10 is probably reasonable.

I can't think of any negative side-effects of clearing id/sequence cache when the pending queue empties.

I would be quite careful of messing with received_tx

Maybe bump the LRU to 100 to be safe.
