I think I figured this one out. I wasn't properly scrubbing the HWADDR, & UUID from '/etc/sysconfig/network-scripts/ifcfg-eth0'.
Closing.

I like when problems solve themselves. :) +1

I'm using the Centos 6.4 from puppet-vagrant-boxes, and this causes the same issue. It looks like lib/beaker/hypervisor/vagrant.rb generates a random base_mac all the time. The centos 64 has a HWADDR specified in /etc/sysconfig/network-scripts/ifcfg-eth0. I'd like to specify a base_mac so that I can ensure it's matched instead of generated randomly. Is this possible? I also would not like to re-package the .box as this would require me to maintain it vs using ones from outside my control, which I like.
Edit:
Actually, this problem is resolved if the Vagrantfile instead of having:
v.vm.base_mac = 'somerandom'
v.vm.network :private_network, ip: "192.168.56.200", :netmask => "255.255.0.0"
has instead:
v.vm.network :private_network, ip: "192.168.56.200", :netmask => "255.255.0.0", :mac => '080027E7C0DF'
Filing new issue.

Hm yeah, we can't override the random mac setting - but if that would be a fix I'm happy to add it.

I just edited my previous comment, it's the generated Vagrant syntax that's causing the issue. See #581
