We could separate the tables in two, one for "dynamic" approaches with the two rows for simple and mutual recursion, and one for the "static" approaches.

Or just leave the "No Explicit Recursion" row of "dynamic" empty.

@serras ; the commit history might be messy, as I iterated over a couple times.

Summary of changes I'd appreciate your green light.

  1. Abstract has been rewritten
  2. Intro has been mildly rewritten in the lights of the new table and
    with the "explicit rec" instead of "deep vs shallow"
  3. Beginning of section "Mutual Recursion" has received a paragraph
    citing previous work on the same indexing technique

I'll implement suggestions you might have tomorrow afternoon and
submit the paper after that!
