Further confusion: The diagrams shipped with the BBB are wrong (GPIO 5 and 4 are reversed).
The correct values seem to be here:
http://beagleboard.org/Support/bone101#headers
Note that beagleboard.org shows P9_17 is GPIO5, and the documentation on the BBB says GPIO5 is P9_18: http://192.168.7.2/Support/bone101/
(192.168.7.2 is my BBB via USB)

It turns out that both the
/lib/firmware/ADAFRUIT-SPI0-00A0.dtbo
and
/lib/firmware/BB-SPIDEV0-00A0.dtbo
overlays were broken.
I used an overlay I found and I was able to work around this problem.
