I ended up using a different tool for this (PyPDF) but had to add extra code in order to make the fields read-only by changing the bit position of the editable form fields.

I had the same issue, though I didn't try to find a solution. I just left the PDF un-flattened. You can probably use another tool or more standard Python library to flatten it properly if you need to.
Following in case the issue is resolved.
