I was a little confused by how Nightwatch works initially too, so hopefully this will help you, or someone else. It's a great tool!
In order:

  1. Follow instructions on the Nightscout website to set-up your own Nightscout monitoring system. This will involve Mlab, Github and Azure set-up, and then sending information from your reader into the cloud. We use a Medtronic 640 and the Enlite sensor, but the process should be similar for Dexcom. At the end of this you should have your own website .azurewebsites.net, and be able to see readings (taken every five minutes) on any web browser worldwide.
  2. Install the latest version of the Nightwatch apk from this website onto your Android mobile. You’ll need to accept the change to your settings to allow installation from a non-recognised source (I switched the control back on immediately after installing). You should now have a new app that you can open up on your mobile.
  3. Make sure that your Android Wear watch has Google Play store installed on it. This threw me, as it does not come as standard with a new Android Wear watch, and mine downloaded it onto the watch later in the day after I thought I’d already finished setting it up. If you don’t have it, try fiddling using Settings – Apps on the watch to get it.
  4. Open up Google Play on your watch, and scroll down to see apps that you have on the mobile that you can also get on your watch. Amongst other useful things like Google Maps, Nightwatch will be one of these. Install it.
  5. Then change your watchface, by holding down on the centre of your watch, pushing to the right, and adding a new Nightwatch watch face. I use the one with small text, which I can read comfortably.

At the end of this you’ll be able to see graph readings on your mobile on the Nightscout website and the Nightwatch app, individual readings on the Nightscout app, and also graphs on your watch via Nightwatch. I’ve found sometimes that Nightscout gets a reading that Nightwatch doesn’t, but a quick force reading on the Nightwatch uploader app fixes that for me.

Sorry if any of the above isn’t completely correct or there are better instructions somewhere else – as a new user I haven’t yet found them!
