What do you think of this interface?

It looks fine as long as the variable names are still editable but the rows could be be less grey and ideally use standard editor colours.

Also the right side of the headers could be an alternative placement for the new buttons.

Worth considering that the performance of this addon could probably be enhanced significantly.

How would that work without changing the runtime behavior of projects?

This just displays that the list is empty in the manager variable, it does not change the execution of the project.

The problem is that not all headers are already displayed so we don't put a button here. But I made the upper area fixed in relation to the scrolling so it is still accessible.
