I could have sworn I had created a LICENSE file ...

This brings up a good question, though. I am a huge fan of the MIT license. Most of the Puppet modules we're using are Apache 2.0 licensed. I'll do some reading to figure out if PuPHPet can be MIT and use Apache 2.0 code, or if it all has to be Apache 2.0.

AL 2.0 is compatible with MIT:
http://www.apache.org/legal/3party.html#criteriaandcategories

Basically I think its best to leave the licenses on modules you are using, but you can apply MIT to whatever you wrote.

Its always a good idea to have the license in each and every file, along with a LICENSE and a NOTICE file.

Personally I like AL 2.0 very much, and my code is using it too. If you use MIT I have absolutely no problem. With GPL it would be different, but it seems it is not an option for you - i am glad about that.

PuPHPet itself has been marked as MIT licensed.

Modules created by PuPHPet organization will be marked as MIT license.

Modules we use are a mixtured of Apache 2.0, GPL and MIT.
