I'm not sure how you'd prefer to conduct the implementation of the TODOs mrdoob but they typically become issues first; even far before they are tackled and patches are sent in. At least this is how my experience has been on GitHub.

If you don't want to clutter the issues then perhaps I'll stop opening issues until I'm actually ready to tackle them immediately but I think trying to conduct it that way will end up with a lot of closed, yet unresolved issues (like the original issue for hierarchy/pivots).

Yeah, I pointed to that file to illustrate that it's an issue I want to tackle at some point. The issue won't be closed until it's been resolved.
