The issue is that OptCuts didn't handle input surfaces with multiple disconnected components if no UV was given. I just did a quick fix in 1904d01, right now it should work, including your input, as long as all the components are in disk topology. Could you please try that and let me know how it works?
I will probably do a more robust fix later.
