I have the mod manager installed in my downloads folder, and completely deleting it and reinstalling worked once and now it's having the same issues no matter what I do.
Don't extract it to a user folder, as this may have permissions that prevent writing files. The exception in your screenshot is an "Access is denied" error, which means the mod manager can't write its own settings file, due to the permissions in the folder you placed it.
Other than that, hotfix 4 today resulted in people experiencing crashes, and it broke the script extender, so all of that is external to the mod manager.
