A-ha, so Windows 3.0 doesn't do the standard 286 vs 386 check with EFLAGS. Instead, it determines 286 vs 386 by executing the SGDT instruction and then looking at the last byte of the output:
SGDT [bp-4]
cmp byte [bp+1],0xFF
; if equal, it's a 286
Apparently 286 processors set the last byte to 0xFF, while all later processors set it to zero.
It has to be the 16-bit version of SGDT.
Once DOSBox-X was updated to emulate this, Windows 3.0 stopped misdetecting the CPU as a 386 and works fine now.
