Clean up Headers about php-reports

Finished most of this. Still need to do the following:

• Add a detail filter so the DETAIL header can be deprecated (or expand the link filter)

• Update the sample report
  

• Make Name and Description use the Info header when placed at the top of the headers like this:

-- My Report Name
-- This is a description
-- of my report

• Trigger an E_USER_DEPRECATED error when a deprecated header is used

Decided to leave the Columns header alone for now since it is the only way to specify multiple filters at a time.
