It looks like ol-infobase0 was not rebooted.
anand@ol-infobase0:/tmp$ uptime 02:58:41 up 24 days, 1:29, 1 user, load average: 0.01, 0.04, 0.00
ol-db was down and services on ol-infobase0 failed. upstart tries to restart a process for 5 times and puts it in failed state after that. The failed services are supposed to be restarted manually.
