This makes me suspect that there's some setting / configuration pigpio.js does that I'm not doing when I start pigpiod.c without any flags.

I think you are on the right track. Can you show the initialization prior to calling gpioInitialise?

Can you provide any other information such as error codes returned by API calls, anything?

Since you mentioned 'concurrent runs': I have a remote ir emitter application currently 'in production'. Concurrency is not practical unless you devise a service through which you funnel all external requests. Essentially you want to protect the internal memory containing the wave ids and not allow a client to clear or clobber them.
