Are you using Symfony 2.1-beta? This might have been caused by #54; however, I no longer have an application using this bundle so any resolution is going to have to come from a PR.

Be sure to use the good branch (2.0 for symfony 2.0.*, or master for symfony 2.1). #54 fixed form generation for the master branch.

Ah, I bet @khepin is using Symfony 2.0 (based on his comment in another repository). In that case, please use the 2.0 branch of this bundle as Emmanuel pointed out. I'll add a note to the readme file about this.
