If you add some text files from Windows and rename them ".S", you get the behavior you expect: with "add type extension" they all get labeled with ".S.txt", without that option they are all simply ".S".
If you do this with actual Merlin sources, the file format converter kicks in and adds its own filename extension, causing all files to be ".S.txt" even if "add type extension" is unchecked. (This was discussed part way through issue #15. This behavior is how SHR images converted to bitmaps end up with the ".BMP" file extension.)
I don't see the behavior described though, where the first file has a different extension from subsequent files. My guess is that it's not a question of first-vs-not-first, but rather the contents of the files being extracted. If the first file did not trigger the Merlin file format converter, but the rest did, you would get the behavior described.
The way to fix this is to prevent the file format converter from setting a file extension unless "add type extension" is checked.
