Could this be because an author joints another team after his work is reviewed? That is, someone is trying to do different topics of the same assignment, one after the other. The system does not support this. A fix might be a rule that you can't leave a team after you've submitted work (or written on the wiki).

Evidently, this is the same as #5, Joining a team causes expertiza to lose everything you've done on your previous topic.

This was probably caused by the bug in accepting an invitation to join a new team, by someone who was on an existing team. This bug was probably fixed in early Sept. 2011 ... though we could not recreate exactly this scenario. (The scenario we could recreate was when all members of a team joined other teams; this led to a "(missing team)" in the list of teams on a particular topic. That has been fixed.)
