As I have said before in #11 , the second point should not be calculated using ImageToGround.

That means, even if the second point (extrusion) is NoData, it should not be NaN.
If you use ImageToGround on the second point, that's akin to the Distance tool, which is not necessarily perpendicular to the ground.

Height above ground should be perpendicular (90deg) to the plane between the viewpoint and the ground point, that is defined by all three heading, pitch, roll values.
