Basically, officially extraing broccoli-middleware from broccoli

• make error handling support both 0.x broccoli errors and 1.x broccoli errors
• talk with jo, and see what else we may have forgotten
• make broccoli package bundle this module by default, rather then implementing it itself

Write integration tests with ember-cli

In order to prevent future regressions with ember-cli such as [this](PR #19 ) we should have basic integration tests in this repo with ember-cli atleast. This helps with releasing new versions of this plugin too.
