I've implemented this in my local version already, but to make it more general decided to call such comments "private". So really it is just a finer granularity of the existing private project and issue concept.
I also had the Reporter Role not able to read private comments by default, and if a role cannot read private comments, all comments they make are also forced to be private (preventing those in such roles from seeing each other's comments).
I'm currently working on making email notifications inline with the viewing behaviour.
