Ok, if you have enough time, can you try to inspect for a more direct attachment saving? I mean by that pushing the stream directly to couch without saving the file on the file system.

I am 80% sure this will require hacking against compoundjs and will therefore be very time-consuming. I'd recommend doing a quickfix with a simple fs.unlink and keep this issue open for when we have more time.

OK, let's go for unlink (in fact it's not compound which is involved it's the bodyparser middleware but you're right it will be probably too much time consuming).

Is it fixed now? I think the "direct stream" should be another issue marked as enhancement.
