In your git repository, the directory .git/hooks has sample hooks and you should install the script here and name it post-receive and mark it executable.

Really old ticket, so sorry for opening it up, but this might help others:

You can also just install the script in ${git_dir}/hooks/ without renaming it. You still have to make it executable, but you can just call it frompost-receive (especially useful if you already have a reasonably large post-receive)

This keeps the post-receive a lot cleaner, since git-slack-hook is quite a large script.
Your post-receive could simply look like this:

#!/bin/bash

# [existing code]

echo "Send notification to Slack.com"
hooks/git-slack-hook
