There is a bug in the last loop of the code. It treats each (example,dimension) pair as if it has its own cluster assignment, which is the wrong way to think about it. There is only one cluster assignment per example.
In particular, the last loop prints out indexes 0-13 of the assignments, which are the cluster assignments for the first 14 examples. Since the dataset has 3 dimensions, example i

corresponds to x->data[i*3+0]
, x->data[i*3+1]
, and x->data[i*3+2]
.
