A possible cause of blank files is here:
EINTR can cause pwrite to return -1, but we don't check the value of errno in bitcask_nifs, but we do return it.

I'm going to close this for now. Although weird and maybe needing work, Bitcask seems to be internally consistent and the error messages were most likely a red herring for the mentioned customer. That is, Bitcask can occassionally see lock files that do not yet contain the pid of the process, so the code can't really delete them at this point. A more standard locking system would be appropriate.
