Totally, there's a lot to do with UI, that's the first next thing for me. Your insights will be much appreciated :)

First, include documentation in the documentation. Five bullet points doesn't cut it, especially these two:
3. Incorporate ad tags in your theme template - How? Nice gist for filters, but no php to add an ad to a template.
5. Configure your ad codes in the WordPress admin - Where? I see nothing about ads in my settings - at least tell me where I should be looking for it

I agree with @webveloper. It is not immediately apparent to me what i'm seeing in the admin interface. I had to crawl through the source code to determine what each of the filters in the gist were doing. Also, it may be helpful to put some visible code in the "acm_output_html" filter example in your sample gist so that users can tell if it's working.. aka something like
$output_html = '<div style="background: red; padding: 20px;">"%url%"</div>';
